Office Policy

• Please be on time.  If you are late it will affect your treatment time.

• Please give at least a two-day notice if you need to cancel an appointment.

• There will be a fee of $100 for a missed appointment or late cancelation.

• If you are planning on using your insurance, please send me a copy of your insurance card and date of birth before your scheduled appointment.
